Understanding your medical history
is important to us!
The Vial of Life is designed to speak for you when you
cannot speak for yourself. The vial contains important
medical information that can assist our emergency
personnel in administering the proper medical treatment.
Help us better help you
It is often difficulty for
people to think clearly during an emergency
situation. Emergency personnel will need to know
many things about you such as your medical
history, allergies, medications, and emergency
contact information.
Often, patients remind us their medical
information is on file at the hospital or their
doctor's office; however this information is not
immediately available to use in the field. Help
us better care for your and/or your loved ones
by completing out a Vial of Life form. This
program is free of charge.
