Release of Medical Information
Your request will be processed within seven working days. We will either mail to the address specified or you may pick up your record at Crivitz Rescue during normal business hours. If picking up records, a photo ID is required. If someone else will be picking up your records, they must have written authorization to pick up your records and must also provide a photo ID.
Billing Information
As part of our efforts to ensure that Crivitz Rescue provides its patients with the best overall service, we have formed a partnership with EMS Management and Consultants Inc. (EMS|MC). EMS|MC is a billing company that specializes in working with EMS agencies across the United States like Crivitz Rescue as well as the patients we serve. If you have a question about a statement that you have received, wish to submit your insurance information, submit your signature for insurance processing, or even pay your bill online please contact EMS|MC:
