Release of Medical Information

To receive a copy of your patient care report, you must complete and sign an authorization to release protected health information. Please be sure to sign the form. Unsigned requests or incomplete forms can not be processed.

Your request will be processed within seven working days. We will either mail to the address specified or you may pick up your record at Crivitz Rescue during normal business hours. If picking up records, a photo ID is required. If someone else will be picking up your records, they must have written authorization to pick up your records and must also provide a photo ID.

Billing Information

As part of our efforts to ensure that Crivitz Rescue provides its patients with the best overall service, we have formed a partnership with with EMS Medical Billing Associates. EMS Medical Billing Associates is a Wisconsin-based company who specializes in working with EMS agencies like Crivitz Rescue as well as the patients we serve.
